24-Hour Crisis Stabilization (Rapid Response Team)

Harmony Healthcare's rapid response team (RRT) is a 24-hour, 7-day per week crisis team. The RRT is physician led and staffed by licensed, trained counselors from a variety of disciplines, including Ph.D.s, LCSWs, CADCs, Nurse Practitioners and MFTs. The RRT meets face-to-face with each and every member in an extreme crisis, as well as members presenting for immediate hospitalization. Once the RRT is notified, the counselor or on call contacts the member within 15 minutes and sets a time for a face-to-face clinical assessment. The assessment can occur in an emergency room, an inpatient psychiatric hospital, intake room, at the work site, or at HHC’s office. Once the assessment is completed, HHC’s staff psychiatrist reviews the medical and clinical data and, in concert with the patient’s treating emergency room physician or PCP, initiates the required treatment. Hospitalized members are seen daily by HHC’s psychiatrist. This hands-on approach enhances the patient’s treatment and encourages effective, ongoing outpatient care post discharge.

After Hours Help Line (Wellplace)

Wellplace, a national, dedicated 365-day a year, 24-hour help line staffed by licensed clinicians and supported by on call supervisory physicians. The 800 number will ensure that your members receive immediate access to care, effective crisis stabilization, proper matching to the most appropriate modality of care, and efficient follow up to make sure that the care your employees and their dependents receive is responsive to their needs.

Onsite Critical Incident Stress Debriefing (CISD)

A team will respond to the workplace or disaster crisis to do CISD. CISD is a crisis intervention process designed to stabilize cognitive and affective processes and further mitigate the impact of a traumatic event.

Case Management/Utilization Review

Harmony’s case management process is initiated at the point of first contact with the client. Regardless of the nature of the case, whether it is a crisis or a regular scheduled session, Harmony Healthcare clinicians register the case with the Harmony case management system.

Harmony’s case management process is designed for immediately assessing and managing:

  • client’s stated preference and understanding of the problem,
  • patient acuity,
  • appropriateness of care,
  • timeliness of services,
  • the clinical and financial efficacy of the approach,
  • nature and quality of support systems, and,
  • discharge needs of client, significant others, and referral sources.

All care is managed by the Harmony Healthcare Case Manager. Inpatient care is monitored daily. Outpatient care is assessed in a multi-disciplinary setting and a treatment plan is formulated which identifies the problems, interventions, objectives and goals. At this time a length of stay or proposed frequency and duration of care is determined. The case is then reviewed based on the proposed care plan. In instances where additional care is anticipated, the case is re-staffed and additional care authorized. Case management review includes pre-admission review, concurrent review, discharge planning and monitoring of aftercare. This methodology assures that all plan participants receive high quality, clinically necessary, appropriate, timely care in a cost efficient manner.

Reporting

Typically, blind statistical reports are requested quarterly outlining the number of patients utilizing the program differentiating between the insured and dependent, diagnosis and treatment modality. No names are used on the reports to protect confidentiality.

Psychiatric Support for Primary Care Physicians

Psychiatric support for network Primary Care Physicians (PCPs) HHC will provide immediate crisis intervention, assessment and consultative services for emergency room and primary care physicians. This responsive service integrates behavioral healthcare with primary medical care.

The result is:

a reduction in the incidents of unnecessary emergency room visits;

a reduction in the need for recurring treatment for medical disorders precipitated by chemical dependency and psychiatric problems;

more efficient, effective use of psychotropics; and
more coordinated and comprehensive care.

This service measurably decreases medical costs and profoundly improves the overall health of participating members.

Internet Access to Behavioral Health Issues

Harmony Healthcare and its Internet partner Wellplace, gives your employees a unique online resource for behavioral health oriented information and services. Through a website created private labeled under your company name and logos, your employees and their families will have access to weekly health related feature articles, daily news briefs, research, assessment tools, forums, and more.

In addition to all the content Wellplace streams into your site, your employees will have exclusive access to Harmony Healthcare clinicians via the Internet. This access will be available via e-mail as well as through a private password protected area of the website. Here, employees can post behavioral health questions to an interactive forum where Harmony Healthcare clinicians will post answers or give appropriate resources for additional information. This is a great way for employees to discuss these questions in total confidentiality privately with professionals or with fellow employees. Often individuals who have questions are shy about posting, however find the same questions asked by others.

Internet Access to Human Resource & Benefit Information

The website that Wellplace will set up for your employees to access behavioral health information can also be customized to include HR and Benefit information provided by your company. Such information could include personnel policies, insurance options, provider lists, company news and message boards. In addition, a specific area could be set up for your employees to access the HR department for the purposes of submitting questions about their benefits. This can be done via e-mail or through an anonymous means.
